Dec 22, 2025: Refinement of diagnoses that flag Transitions of Care as Behavioral Health

We have refined down the list of diagnoses that trigger a Behavioral Health flag to be added to a Transition of Care.

What's Changed

Previously, Transition of Care (TOC) Lenses include a resource.extension with "url": <https://zusapi.com/lens/extension/behavioralHealth"> that is true when any Condition from the aggregated encounters has a "CCSR" code starting with MBD(Mental, behavioral, neurodevelopmental disorders).

The goal of this extension is to help care teams identify hospitalizations that are Behavioral Health in nature to help tailor outreach and appropriately trigger clinical workflows.

As such, we've made an update to the codes that will mark a TOC as a Behavioral Health related one, removing ICD-10 codes associated with Tobacco use/dependence and intellectual disabilities.
