July 2, 2026: Distinct ED visits are no longer merged into a single Transition of Care

Separate visits that happen to reuse a visit number are no longer grouped into one Transition of Care.

Previously, Zus pre-grouped source encounters by an internal visit number (a combination of the encounter's identifiers) before applying the time-window grouping. Some sources reuse the same visit number across unrelated encounters months apart (for example, certain clinical-summary feeds). That caused genuinely distinct visits to be collapsed into a single episode — producing Transitions of Care with implausibly long lengths of stay and hiding separate admissions or ED visits.

Zus now groups encounters into Transitions of Care using the time window alone (plus the readmission split, where enabled); the visit-number pre-grouping has been removed. Distinct visits now form distinct episodes with accurate boundaries and lengths of stay. As a related improvement, encounters whose identifiers differ only in order are now treated as unchanged, so they no longer trigger spurious updates. No changes are required on the customer side.
