Nov 20, 2025: Clearer Hospitalization data in the ZAP

We've made updates to how Hospitalizations appear in the Overview section of the ZAP. These changes provide a clearer, more complete understanding of a patient's hospitalization history while simplifying the overall visualization.

What’s New

1. The Hospitalization Overview now displays all relevant episodes of care

Previously, the Overview only surfaced Emergency and IMP hospitalizations. We’ve expanded this to include additional visit types: OBSENC and PRENC so care teams have a complete view of all types of hospitalization visits now including Observation and Pre-admission visit types.

2. New visual indicators highlight critical events

In the Hospitalization Overview list, we now display icons signaling when the hospitalization includes any of the following:

  • Behavioral health–related admission
  • Readmission
  • Non-elective admission (direct, holding, or emergency)

These indicators help care teams quickly identify high-risk or unexpected admissions and key off of recent changes we made to our Transition of Care Lens data.

3. Better visibility into relevant documents

Clicking a row in the Overview card opens a drawer that displays all documents related to the hospitalization. This improves visibility into document types, including discharge narratives and encounter-level documentation.

Why This Matters

These updates improve clarity and ensure that high-value clinical signals like readmissions and behavioral health events are visible at a glance.
