Sept 5, 2025: Track patient hospitalizations more easily in the Zus App

Starting today, the Transitions page of the Zus App includes additional filtering options and data taking advantage of recent updates to the Transition of Care (TOC) Lens.

Quick views can help narrow hospitalizations to focus on certain types
Users can now select a quick filter at the top of the page. These quick views will filter the list in addition to the existing filters on date, etc. There are four available quick views:
- All- View all hospitalizations.
- Planned - A hospital admission that does not appear to be admitted from another source and was most likely elective.
- Unplanned - A hospital admission that Zus was able to discern was non-elective coming from an emergency department, direct admission, or from an observation.
- Behavioral Health - A hospitalization is categorized as "Behavioral Health" if it has an associated condition has a "CCSR" code starting with MBD (Mental, behavioral, neurodevelopmental disorders).
Additional details for each hospitalization available
When users click on hospitalization, there is some new detail available in the side panel:
- If admitting diagnoses are not available, a "Reason for Visit" will display if it exists
- "Current Participants" indicates the entire list of individuals associated with the hospitalization. This may include providers, hospital staff, or other relevant persons involved with the hospitalization.
- These details are also now included in the table on the Transitions page
