Patient History Request

Overview

The Patient History Request form allows providers to manually request or refresh patient data in Zus.

What data is requested?

The Patient History Request form will place a request with Zus's Patient History API, querying the data sources included in your Zus data package (e.g., CommonWell, Carequality, and Surescripts). Even when querying from a specific component (e.g., Conditions), the request retrieves data across all clinical data types.

Who can I request data for?

Data can be requested by provider organizations and pharmacies for patients with whom there is a Treatment Purpose of Use.

What information do I need to provide?

• Practitioner information: Name, role, and individual (not group) NPI for a provider overseeing the patient's care.
• Patient demographics: This will be auto-populated with any information already in Zus. If changes are made and a request is submitted, the demographic changes will be saved. For more information on required fields, see the Patient History API details.

How often is data refreshed?

This form enables manual requests and refreshes. Many of our customers have automated this process by integrating with Zus APIs. Additionally, Zus is working on regular time- or trigger-based data refreshes, which should be available in Q2. Please contact support if additional questions in the interim.
